Their community nursing teams support house-bound residents with a wide range of mental and physical health needs, from wound management and diabetes to kidney failure and heart or lung conditions.
They work closely with GPs, hospital staff, consultant specialists and social care workers to identify a person’s individual care needs and co-ordinate the different services they receive.
